Jake Young

Founder, FormDr

Jake is a San Francisco-based-Internet entrepreneur and founder of the web application, FormDr. FormDr lets patients complete medical forms, from any device, before their appointment.

With more than two decades of internet marketing experience, Jake has worked with some of the largest healthcare systems, including CHRISTUS Health and St. Luke’s, and helped hundreds of practices bring FormDr to their patients.

Prior to founding FormDr, Jake founded Siteplicity, a digital marketing company, to simplify the web design experience for small businesses and private practices.

Formerly, Jake was the lead interface designer for The Department of Defense’s TroopTube social network. His work has been featured on global publications including: The Guardian, TechCrunch, Wired, and NBC News; and, it has been awarded by the American Advertising Federation.

In his free time, Jake has a permanent travel bug and enjoys spending weekends at the park with his English bulldog, Kimber.

Jake earned his Bachelors of Fine Arts in Interactive Media Design from The Art Institute.
